Junior Academy Liability Waiver & Terms of Enrollment

As the parent or legal guardian of the player named above, I understand and agree to the following terms in accordance with the laws of the Province of Alberta, Canada:

1. Acknowledgment of Risk

I understand that participation in cricket training and related activities carries inherent physical risks, including but not limited to minor injuries, muscle strains, or unforeseen accidents. Pursuant to the Occupiers' Liability Act, RSA 2000, c O-4, and the principles of voluntary assumption of risk, I voluntarily and knowingly accept these risks on behalf of my child. I acknowledge that the academy takes reasonable steps to ensure participant safety.

2. Release & Waiver of Liability

To the fullest extent permitted by law in Alberta, I agree to release, indemnify, and hold harmless Pitch N Play Junior Academy, its coaches, staff, volunteers, and affiliates from any and all claims, damages, losses, or liability arising from my child's participation in any training sessions or academy events. This release does not apply in cases of gross negligence or intentional misconduct on the part of the academy or its staff. I acknowledge that pursuant to the Limitations Act, RSA 2000, c L-12, certain limitation periods may apply to any claim not released herein.

3. Emergency Medical Consent

In the event of an injury or medical emergency during training or an academy event, I authorize the academy staff to seek and obtain appropriate emergency medical care for my child, including contacting emergency services (911), as permitted under Alberta's Emergency Medical Aid Act, RSA 2000, c E-7.9. I accept financial responsibility for any associated medical costs not covered by applicable provincial health coverage. I confirm that any health information provided on this form may be shared with emergency responders as necessary.

4. Media & Photography Consent (PIPA)

In accordance with Alberta's Personal Information Protection Act (PIPA), SA 2003, c P-6.5, I give my informed consent for Pitch N Play Junior Academy to collect, use, and disclose photographs and/or videos of my child taken during sessions or academy activities for promotional, educational, and social media purposes, without monetary compensation. I understand that I may withdraw this consent at any time by providing written notice to the academy, and that withdrawal will not affect materials already published.

5. Personal Information & Privacy

I consent to the collection and use of personal information (including my child's name, age, contact details, and health information provided on this form) by Pitch N Play Junior Academy solely for the purposes of program administration and communications, in compliance with PIPA. Personal information will not be shared with third parties without consent except as required by law.

6. Parent/Guardian Confirmation

I confirm that I am the parent or legal guardian of the player named above and have the legal authority to sign this waiver on their behalf. I have fully read and understood this waiver and agree to abide by the academy's code of conduct and all applicable rules and regulations. I acknowledge that this waiver is governed by the laws of the Province of Alberta and that any disputes shall be resolved in the courts of Alberta.

Note: If your child is 18 years of age or older, this waiver may be signed by the participant directly.
